Blood Cancer Patient Survey 2019

This questionnaire is for you to share your experiences of the impact of blood cancer. Its purpose is to understand the emotional and psychological impact and how well your needs are being met.

Taking part in this survey is voluntary. Published reports will not contain any personal details.

Who should complete the questionnaire?
The questions should be answered by you, as the person diagnosed. If you need help to complete the questionnaire, the answers should be given from your point of view – not the point of view of the person who is helping.

For each question please select the box that is closest to your views. For some questions, you will be instructed that you may select more than one box. Don’t worry if you make a mistake; simply put a tick in the correct box.

IMPORTANT INFORMATION

The information is being collected for Bloodwise and Anthony Nolan by Quality Health, an experienced survey provider. To make sure the information we collate is useful, we need to collect some personal details from you. The purpose of collecting this information is to generate aggregated statistics about the care and treatment people receive.

By completing the questionnaire, you are giving your consent for the information you provide to be used for the above purposes.

No identifying information will be included in any publications or presentations based on these data, and your responses to this survey will be anonymous. Please note that any comments you provide will be looked at in full by the researchers analysing the data. We will remove any information that could identify you before publishing any of your feedback.

You can withdraw the information you give in this questionnaire upon request, up to the point at which data are analysed and personal details are removed (18 October 2019).
