Sarcoma UK Family and Carers Survey
This questionnaire is about your experience as a family member or carer for someone with sarcoma. Its purpose is to provide information, which can help Sarcoma UK monitor and improve the future quality of health services for people with sarcoma. If you are bereaved, we would still appreciate your views if you feel able to share them.
The survey is being carried out by an experienced health research company, Quality Health, on behalf of Sarcoma UK.
Taking part in this survey is voluntary. Published reports will not contain any personal details.
Who should complete the questionnaire?
This questionnaire should be answered by you, as the family member or carer of a person with sarcoma. If you need help to complete the questionnaire, the answers should be given from your point of view – not the point of view of the person helping.
Completing the questionnaire
For each question please select the option that is closest to your views. If you make a mistake, simply click inside the box you meant to tick. The response should move. If the question has multiple responses, clicking on a tick again will remove it. Please do not write your name or full address anywhere on the questionnaire.
Sarcoma UK will NOT receive your name and address unless you consent at the end of the questionnaire to further contact. The data collected will be held and handled securely, anonymised after analysis and before any publication.

To make sure the information we collect is useful, we need to collect some personal details. This is to form an overall picture about the care and treatment people receive, and to understand what may be causing any differences.
By completing this questionnaire, you are giving your consent for the information provided to be used for the above purposes. Specifically, you are agreeing that:
- Your personal details and relevant health information can be held and used by Quality Health and Sarcoma UK to analyse the data
- Your responses will be viewed and analysed by researchers at Quality Health and Sarcoma UK
- Your personal information will not be released by anyone working on behalf of Quality Health or Sarcoma UK unless required by law or where there is a clear overriding public interest
- Please note that the free text / comments you provide will be looked at in full by researchers analysing the data and SUK. We will remove any information that could identify you before publishing any of your feedback
